  1. Зачем malloc, когда есть new? Мы на плюсах уже третий месяц пишем :)
  2. Твои заголовочники должны быть "самодостаточными", т.е. не зависеть от инклюдов, которые стоять до них. А у тебя этого не происходит. my_vector_impl.h использует malloc и cout, при том что заголовочные файлы ты не подключил. У тебя это компилируется, потому что нужные заголовочники добавлены до инклюда этих файлов. Но вот наших тестах этого не сделано и компиляция упала.
  3. Не нужно писать using namespace в заголовочных файлах. Тот код, в который они будут подключаться, вряд ли обрадуется, когда им в глобальное пространство имен залетит вся стандартная библиотека без боевого предупреждения. Если очень хочется сделать using namespace, то делай это в пределах своего неймспейса, а не в глобальном пространстве.
  4. Дебажного закомменченого кода в посылке быть не должно!

Итак, после того как я пофиксил всю твою компиляцию (больше так делать не буду), у тебя прошо меньше половины тестов, а большинство попадало на сегфолтах и с утечками валгринда.

Утечки валгринда объясняются тем, что вызов деструкторов для элементов у тебя закомменчен почти везде
Еще ты копируешь элементы оператором присваивания. В общем случае класс T не обязан его иметь, контракт вектора этого не требует, он требует только конструктора копирования.
Гарантий по исключениям тоже никаких не выполняется.
